Gauge could be bad, sensor could be bad or Engine control module (ECM), which would control caution messages, could have a problem. A functioning gauge will usually read 210F as I mentioned above. Not sure if one of those new probe cameras can be inserted into the output port of the water pump to inspect the vanes...you might have to unbolt the output nipple/nozzle to do the inspection. Have you inspected the radiator core?

If you are driving slowly or stopped idling the fans keep enough air moving through your radiator to keep your engine cool.
